A Legal Disclaimer

This Terms and Conditions Agreement (“Agreement”) is a binding agreement between you (“Client,” “you,” or “your”) and Ask Nurse Susan, LLC (“Ask Nurse Susan,” “we,” “us,” or “our”). By scheduling or participating in a virtual visit or otherwise using our services, you agree to the terms of this Agreement. If you do not agree to these terms, you may not access or use the services.

Description of Services

Ask Nurse Susan, LLC provides virtual nurse advise designed to offer general health guidance and education. Services are provided via Zoom or telephone and include:

• 15-Minute Quick Advice ($25): For one specific health question or concern.

• 30-Minute Health Guidance Session ($50): For more complex or ongoing issues.

• 60-Minute Comprehensive Health Review ($90): For a complete health review and care planning.

These sessions are for informational and educational purposes only and are not a substitute for professional medical diagnosis, treatment, or emergency care.

Appointment Scheduling and Payment

Appointments are scheduled through our online booking system. Payment is due at the time of booking and may be made via credit card, PayPal, or Venmo. Ask Nurse Susan, LLC does not accept insurance, Medicare, or Medicaid.

You may cancel or reschedule your appointment up to one (1) hour before the scheduled start time for a full refund or credit. Cancellations made less than one hour prior to your appointment, or failure to attend (“no-show”), may result in forfeiture of the session fee.

Private Pay Notice

All sessions with Susan O’Connor, RN are private-pay services focused on nurse-led health guidance and education to help clients better understand their health and make informed decisions. These sessions are not medical treatment and are not eligible for insurance or Medicare/Medicaid reimbursement.

Rescheduling and Missed Appointments

Privacy and Confidentiality

Ask Nurse Susan, LLC respects your privacy. Notes and summaries from your visit (“Client Records”) may be retained securely for the purpose of providing continuity of care, including follow-up communication or “Next Steps” recommendations.

Client Records are kept confidential and will not be shared with any third party without your consent, except as required by law. No video or audio sessions are recorded without express written permission from both parties.

Disclaimers

No Nurse–Patient Relationship. Your use of our services does not create a nurse–patient relationship. Our consultations provide general health education and guidance only and are not a substitute for medical care from a licensed healthcare provider. We do not diagnose, treat, or manage medical conditions. Ask Nurse Susan, LLC provides general nursing guidance and education only. We do not provide medical diagnosis, treatment, prescriptions, or emergency care.

You acknowledge and agree that:

- You are solely responsible for any decisions or actions taken based on information provided during your session.

- You understand that this service is not a substitute for care from a licensed physician or other healthcare provider.

- If you experience a medical emergency, you must call 911 or seek immediate medical attention.

All services are provided “as is” and “as available,” without any warranties of any kind, express or implied, including but not limited to merchantability, fitness for a particular purpose, or non-infringement.

Limitation of Liability

To the fullest extent permitted by law, Ask Nurse Susan, LLC and its representatives shall not be liable for any indirect, incidental, consequential, special, or punitive damages arising from or related to the use of its services.

In no event shall the total liability of Ask Nurse Susan, LLC exceed the amount paid by you for the specific consultation giving rise to the claim.

Governing Law and Dispute Resolution

This Agreement shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law provisions. Any disputes arising from this Agreement shall be resolved in the state or federal courts located in Florida.

Service Availability & Licensing Notice

Ask Nurse Susan, LLC provides general health guidance and educational information only. Although these services do not constitute medical diagnosis or treatment, we maintain compliance with state regulations regarding professional nursing services.

At this time, our services are not available to clients located in the following states or jurisdictions:

Oregon, Nevada, New York, Alaska, District of Columbia, Hawaii, Massachusetts, Pennsylvania, Washington, and Minnesota.

By scheduling a session, you represent and warrant that you are physically located in a state where we are permitted to provide services. If we become aware that you are located in a restricted state, your session will be canceled and refunded

Modifications

Ask Nurse Susan, LLC reserves the right to modify these Terms and Conditions at any time. The most recent version will always be posted on our website. Your continued use of our services after changes are made constitutes acceptance of the revised terms.
